We’re looking for a Service Coordinator to join our team and play a key role in supporting families in the Northern Suburbs (maternity leave cover).

About the role
- Coordinate services and case management for children and families.
- Manage client onboarding, including liaison with families and external providers.
- Complete and maintain all required documentation such as care plans, service agreements and risk assessments.
- Maintain regular client contact, including home visits and ongoing review of supports.
- Provide supervision and day‑to‑day leadership to Support Workers.
- Work with the rostering team to manage staffing changes and ensure appropriate staff allocation.
- Support staff performance, problem solving, mentoring and participation in recruitment activities.
- Assist the Director Clinical Operations with operational planning, continuous improvement and professional development requirements.
About you
- Experience supporting clients in the disability and/or health sector
- Working knowledge of the NDIS.
- Experience supervising and leading staff.
- Strong analytical and problem‑solving skills.
- Highly competent with technology and able to learn new systems quickly
- Strong communication and interpersonal skills
- High attention to detail with a commitment to continuous improvement.
- Ability to work independently, manage priorities and adapt to change.
